Welcome back! If you would like to schedule an appointment with Dr. Derik, simply click on “schedule an appointment” below.
If Dr. Sanders hasn’t seen you in over two months or you have a new injury, please print off and fill out the Return Patient paperwork to bring to your appointment and be sure to book the longer appointment time (30 min). If you don’t have time to fill it out before your appointment, plan to arrive 15 minutes early to complete it.
